Medical Info Form - Filled out and signed by Parent
Medical Clearance Form -Filled out and signed by Doctor.
One school picture or wallet size photo. Must also be uploaded when registering.
Copy of school report card (year-end report card/4 quarters).
Copy of Birth Certificate
Undated check #1 - $150 for equipment deposit (Tackle)
Undated check #2 - $100 for work bond (Tackle)
Fundraising participation per child (Flag & Tackle)
