- Participant Contract/Parental Consent Form - Fill out page 2 of this form.
- Medical Info Form - Filled out and signed by Parent
- Medical Clearance Form -Filled out and signed by Doctor.
- One school picture or wallet size photo. Must also be uploaded when registering.
- Copy of school report card (year-end report card/4 quarters).
- Copy of birth certificate
- Undated check #1 - $150 for equipment deposit (all cheer)
- Undated check #2 - $100 for work bond - EXCEPTION FLAG
- Fundraising participation per child (all cheer)
